To submit an application for the acupuncture program, please use our online application form below. Please note that there is a non-refundable $100.00 application fee which must be paid before submission of any transcripts.

Step 2: Submission of all transcripts require a non-refundable $100.00 registration fee. Once you have filled out the application please return to this page and pay below:
If using paypal.me, please specify that the $100 is for the Application Fee and enter your name in the notes field.
Step 3: You must have sealed official transcripts sent from each school you list under the prior education section of this application. The transcripts should be mailed to the MWAI Admissions Office or emailed to firstname.lastname@example.org. Please list all accredited college and university-level institutions you have attended including technical schools in chronological order on the application.
