Chinese Herbal Medicine Certificate Program Application
To submit an application, please use our online application form below. Please note that there is a
non-refundable $100.00 application fee (Fee waived until further notice) which must be paid before submission of any transcripts.
Step 1: Please fill out the form on this page, after you click the submit button you will be redirected to a portal site to complete the application.
Step 2: Submission of all transcripts require a
non-refundable $100.00 registration fee (Fee waived until further notice). Once you have filled out the application please return to this page and pay below:
If using paypal.me, please specify that the $100 is for the Chinese Herbal Medicine Certificate Program Application Fee and enter your name in the notes field.
Step 3: You must have sealed official transcripts sent from each school you list under the prior education section of this application. The transcripts should be mailed to the MWAI Admissions Office or emailed to firstname.lastname@example.org. Please list all accredited college and university-level institutions you have attended including technical schools in chronological order on the application.

The Certificate in Chinese Herbal Medicine program offered by Middle Way Acupuncture Institute is approved to begin enrolling students but is not accredited or pre-accredited by ACAHM. This program is eligible for ACAHM accreditation, and Middle Way Acupuncture Institute is currently in the process of seeking ACAHM accreditation for the program. However, the Institute can provide no assurance that accreditation will be granted by ACAHM. Graduates of an unaccredited program are not considered to have graduated from an ACAHM-accredited program and may not rely on ACAHM accreditation for professional licensure or other purposes.
